name and amount of the medication, and price
are placed on the card. The purchased of OTC

serves as a complete record of medication
obtained by each member of this family. Any
special information concerning the patient, such
as allergies to particular medications or the
presence of a particular disease state should be
recorded.
THE PATIENT OR FAMILY RECORD CARD-
Having this information. the pharmacist will be
able to defect problems related to drug I therapy
readily. Such problems may arise when a patient
is seeing more than one [physician or is taking a
medication that is contraindicated in a particular
disease state or that might interact with other
prescription or OTC medication that is being
taken simultaneously. The use of this method
also permits rapid location of prescription orders
without the need of either number or date, and
facilities the supplying of a listing of all |
medication dispensed for income tax or insurance
purposes.
THE PATIENT OR FAMILY RECORD CARD-
PRESCRIPTION FOR CONTROLLED SUBSTANCES
MUST BE FILED. IN EITHER ONE OF THE THREE
WAYS:
1. A pharmacy can maintain three separate files:
A file for
Schedule 11 drug dispensed, a file for Schedule
III, IV and V drugs dispensed, and a file for prescription
for all other drugs.
2. A pharmacy can maintain two files:
a file for all
Schedule Il drugs dispensed and another file for all other
drugs dispensed including those Schedules III, IV and V.
